Reimbursement Form

Student participants in the University Undergraduate Research and Arts Forum may request reimbursement up to $25 to cover the cost of preparing a presentation for the Forum. In order to be reimbursed, download and complete this reimbursement form. It is not necessary to provide receipts. Please round off amounts to the nearest dollar.

Applications must be submitted no later than April 21, 2008 to 105 Eustace-Cole Hall.
